Look Boutique Salon Covid-19 Client Waiver Form

We at Look Boutique Salon are doing everything we can to protect you, our clients, as well as our staff. To this extent, we will be following the Center for Disease Control (CDC), local health department and the Virginia State Board for Barbering, Cosmetology and Esthetics guidelines with regard to social distancing practices, as well as the cleaning and disinfection of our salon in order to reduce the spread of the Novel Coronavirus, or COVID-19. This will require our staff to maintain six (6) feet of distance between ourselves and you, the client, as much as possible unless performing your service. We will also be requiring this same procedure for client-to-client contact in order to limit the exposure to all individuals.

We will require all individuals (staff and clients) to utilize either a surgical mask or a cloth mask that covers both your mouth and nose at all times to reduce the risk of exposure to yourself and others while in the establishment. We can provide a mask if you do not bring one for a small fee. Please note that while wearing your mask during hair services there is a risk of staining and/or color damage to it and Look Salon LLC is not liable to replace any masks if such damage occurs, so please make sure you wear one that you do not mind getting ruined.

It is required of everyone to either wash or sanitize their hands upon entry into the salon, after using the restroom or touching their face or mask. Please limit any bags or purses you bring to one that can hang on our hook at the styling stations and we ask that you do not bring extra bags with you. We will be temporarily halting our beverage offerings, such as coffee or water, but you may bring your own.

When arriving for appointments please wait outside (we have several chairs spaced apart) until your stylist has completed disinfecting their station and is ready for you to enter.

We are not able to offer beard trims or makeup services during this time.

We are temporarily unable to accept cash payments and ask to limit payments by check.

We are using a clean cape on every client as well as wearing a new apron, a mask and, at times, a face shield. We have implemented touch free automatic soap and hand sanitizers in the salon for your use.

By signing you agree to this waiver, you attest that you are clear of the following symptoms and situations within the last 14 days leading up to your appointment.
* Fever of 99.6 degrees or above, or possible fever symptoms such as chills and/or sweating
* Cough
* Trouble breathing, shortness of breath, wheezing
* Muscle aches
* Sore throat
* Diarrhea
* Loss of smell or taste, or a change in taste
* Headache
* Have had contact with an individual who has shown signs of the above symptoms within the past 14 days
* Have been around or caring for anyone with confirmed COVID-19 within the past 14 days

Upon entering the building, Look Boutique Salon reserves the right to take your temperature using our infrared thermometer. If you have a temperature of 99 degrees or above, we reserve the right to refuse service and ask that you leave immediately. You can rebook your appointment after 14 days or by providing proof of a negative COVID-19 test.

At this time, per the State of Virginia mandate, we can only accept clients with an appointment.

At any time during our contact with you, if we feel you are not complying with the rules stated above, show signs of illness or in any way do not comply, we reserve the right to refuse service.

If we all work together, we can overcome the spread of this virus. We, at Look Boutique Salon, are doing everything we can to ensure a comfortable and welcoming environment while staying as safe as possible. We are happy to welcome our clients back!

Agreeing to this waiver you understand that by entering a business open by appointments to the public you are susceptible to the risk of exposure to any illness including. but not limited to, the Coronavirus, also known as COVID-19, and will not hold Look Salon LLC liable for any symptoms of illness following your contact with us. Furthermore, you also agree to contact us should you develop any symptoms within 14 days after your visit.
